OPTIMISTIC is a model of care that embeds a collaborative clinical team in nursing facilities to optimize chronic disease management, reduce unnecessary hospitalizations, and help residents develop and meet their goals of care. Bright Orange Thread, an online marketing agency, was working with the OPTIMISTIC team and contacted PROSPER to collaborate on the design of a website. PROSPER designed an attractive, user-friendly website where OPTIMISTIC could communicate its vision as a catalyst for change.
